Foundayo is orforglipron, Eli Lilly's once-daily GLP-1 pill, approved by the FDA on April 1, 2026 for weight management. It is different from everything else on this site in one way that matters at 7 AM: it is a small molecule rather than a peptide, so it is absorbed like a normal pill and can be taken with or without food. No empty stomach, no sip of water, no 30-minute wait. This review covers what it did in trials, what it costs, the side effects, and the trade it asks you to make.

What Foundayo Is and Who It Is For

Foundayo is approved for adults with obesity, or overweight plus at least one weight-related condition, alongside a reduced-calorie diet and more activity. It comes in 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg tablets. You start at 0.8 mg, move to 2.5 mg after at least 30 days, then 5.5 mg after at least another 30, and from there your prescriber may take you to 9, 14.5 or 17.2 mg depending on response and tolerability. The maximum approved dose is 17.2 mg once daily.

What It Did in Trials

Worth knowing: the trials studied doses up to 36 mg, but the FDA approved a maximum of 17.2 mg. Real-world results at the approved doses may differ from the trial headline, in either direction.

Cost in 2026

Lilly priced Foundayo to compete directly with the Wegovy pill. Self-pay as of October 2026 is $149 for the 0.8 mg and 2.5 mg doses, $199 for 5.5 mg, and $299 for 9, 14.5 and 17.2 mg when you refill within 45 days; without the refill discount the top doses run up to $349. Eligible Medicare Part D members may pay a $50 monthly copay through the GLP-1 Bridge program that began July 1, 2026, which covers Foundayo alongside Wegovy and the Zepbound KwikPen. Commercial coverage is being added plan by plan. Check LillyDirect for current numbers.

Side Effects

The familiar GLP-1 list: nausea, vomiting, constipation, diarrhea, reflux and reduced appetite, most noticeable around dose increases. Because the drug is taken daily rather than weekly, some people find the pattern steadier and less peaky than an injection; some do not. The label carries serious-risk warnings consistent with the class. Food manages the common effects; the GLP-1 nausea guide and constipation guide cover that. Everything else is a conversation with your prescriber.

Eating on Foundayo

Because Foundayo does not dictate when you eat, you get to decide, and the right decision is the same as on every GLP-1: a small, protein-first breakfast instead of skipping it, five small eating windows, protein before anything else on the plate, and a daily floor near 100 grams so you keep your muscle while the fat comes off. The GLP-1 protein food list and 7-day GLP-1 meal plan are built for appetites this small.

Foundayo vs the Alternatives

How People Actually Get Foundayo in 2026

There are three routes, and the right one depends on your insurance and your diagnosis.

  1. Through your own doctor and insurance. Foundayo is labeled for weight management; plan coverage is still rolling out, so ask your prescriber and your plan. Ask your prescriber what your plan requires; prior authorization is common.
  2. Self-pay direct from the manufacturer. LillyDirect sells Foundayo at the $149 to $299 self-pay tiers (up to $349 without the 45-day refill discount). Prices change often, so check the manufacturer's page before you decide.
  3. Through an online weight-loss clinic. A licensed clinician reviews your history by video or questionnaire, prescribes if appropriate, and either sends the prescription to a pharmacy or ships treatment to you. This is the fastest route if your regular doctor will not prescribe for weight loss.

The Part No Prescription Fixes

Every drug on this page works the same way at the dinner table: it shrinks your appetite until three or four bites feel like a meal. That is where the weight loss comes from, and it is also where the muscle loss comes from if those bites are toast and crackers instead of chicken and eggs. The medication does not care what you eat. Your body does.

HealthyOne was built for exactly this. Say what you ate or snap a photo, and it logs the protein before you have cleared the plate. GLP-1 mode raises your protein target to protect muscle while the weight comes off, the dashboard shows protein first, and the 50-plus nutrient tracker underneath flags the iron, fiber and B12 that slide when every meal is tiny. Start with the GLP-1 protein food list and the 7-day GLP-1 meal plan.

FAQ

How much weight do people lose on Foundayo?

In ATTAIN-1, adults without diabetes lost 11.1 percent of their body weight on average across everyone enrolled at 72 weeks, versus 2.1 percent on placebo. Among those who stayed on the highest trial dose, the loss was about 12.4 percent. In ATTAIN-2, people with type 2 diabetes lost about 10.5 percent at the highest dose.

Do you have to take Foundayo on an empty stomach?

No. Foundayo is a small-molecule drug that is absorbed normally, so it is taken once a day with or without food and without the water and waiting rules the Wegovy pill requires. That is its main practical advantage.

How much does Foundayo cost?

Lilly's self-pay pricing as of October 2026 is $149 a month for the 0.8 mg and 2.5 mg doses, $199 for 5.5 mg, and $299 for 9, 14.5 and 17.2 mg when refilled within 45 days; without the refill discount the upper doses run up to $349. Eligible Medicare Part D members may qualify for a $50 copay through the GLP-1 Bridge program.

Foundayo or the Wegovy pill?

The Wegovy pill lost more weight in its trial (13.6 versus 11.1 percent) but must be taken on an empty stomach with a 30-minute wait. Foundayo can be taken any time, with food. If you are confident you will keep the morning routine, the Wegovy pill has the stronger numbers; if you are not, the pill you actually take wins.

The Bottom Line

Foundayo is the easiest GLP-1 to live with: one pill a day, no needle, no empty-stomach rules, at $149 to $349 a month. In exchange it produced less weight loss in trials than the Wegovy pill and considerably less than the injections. For a lot of people that is the right trade, because the drug you take every day beats the drug you abandon. Whatever you choose with your prescriber, the appetite will drop and the protein will not take care of itself.
