Two years ago this comparison had two real entries. Now there are six FDA-approved options, two of them daily pills, and the cash prices have come down enough that people without coverage are paying for them. This is the plain-language version: what each one is, what it did in trials, what it costs when you pay yourself, and who it tends to suit. Nothing here replaces a conversation with your prescriber; it just makes that conversation shorter.

One thing to settle up front. These drugs are four molecules under six brand names. Semaglutide is Ozempic, Wegovy and the Wegovy pill. Tirzepatide is Mounjaro and Zepbound. Orforglipron is Foundayo, the newest. The brand name mostly tells you what the FDA label says it is for, and the label is what your insurance reads.

Trial figures are average percent body-weight loss across everyone enrolled unless noted, from each drug's pivotal trial (SURMOUNT-1, STEP 1, STEP UP, OASIS 4, ATTAIN-1). Cash prices are the manufacturers' published self-pay programs as of October 2026 and change often.

Semaglutide vs Tirzepatide: The Only Head-to-Head

Most comparisons stack numbers from different trials, which is unreliable. There is one direct test: SURMOUNT-5, published in 2025, put Zepbound against Wegovy 2.4 mg for 72 weeks in adults with obesity and no diabetes. Zepbound averaged 20.2 percent weight loss against 13.7 percent for Wegovy. About 65 percent of the Zepbound group lost 15 percent or more of their body weight, versus 40 percent on Wegovy. Dropouts for side effects were similar, around 6 to 8 percent in each group.

Two caveats. The trial used Wegovy 2.4 mg; the 7.2 mg HD dose approved in March 2026 reached about 20.7 percent in its own trial, so the gap at the top end is now much narrower. And averages hide a lot. Plenty of people lose 25 percent on semaglutide and plenty plateau at 8 percent on tirzepatide. Our post on why some people lose more on a GLP-1 covers what tends to separate them, and most of it is food.

Shot or Pill

Until 2026 this was not a choice. Now it is. The Wegovy pill is the same semaglutide in a daily 25 mg tablet that has to be taken on an empty stomach with a small sip of water, then nothing for about 30 minutes. Foundayo is a different molecule built to be absorbed normally, so it is taken once a day with or without food, which is a real quality-of-life difference.

Both pills lose less weight on average than the shots. Both cost less. For someone who would never stick with injections, a pill they take is better than a shot they skip. For someone who has already done well on a weekly injection, there is no reason to switch.

What They Cost When You Pay Yourself

This is where 2026 changed the picture. Novo Nordisk's self-pay price for Wegovy and Ozempic is $349 a month, with a $199 introductory price on the two lowest doses for new patients, and the Wegovy pill runs $149 to $299 by dose. Lilly's LillyDirect program prices Zepbound vials and KwikPens at $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 to 15 mg when you refill within 45 days. Foundayo is $149 to $299, or up to $349 at the top doses without the refill discount. Medicare Part D members may qualify for a $50 monthly copay on Wegovy, Foundayo and the Zepbound KwikPen through the GLP-1 Bridge program that began July 1, 2026.

Mounjaro and Ozempic are priced and covered as diabetes drugs. If you have type 2 diabetes, they are usually the easiest to get covered. If you do not, Zepbound and Wegovy are the same molecules with the label your insurer wants to see.

Side Effects, Briefly

Every drug here shares the same list: nausea, constipation, diarrhea, reflux, fatigue, and a loss of appetite that can tip into under-eating. They are worst in the weeks after a dose increase and they are mostly managed with food, which is what the rest of this site is about. Start with what to eat for GLP-1 nausea and GLP-1 constipation. Anything beyond that, including whether to pause or change dose, is a question for your prescriber, not a blog.

Who Each One Tends to Suit

How People Actually Get These Medications in 2026

There are three routes, and the right one depends on your insurance and your diagnosis.

  1. Through your own doctor and insurance. Zepbound, Wegovy, the Wegovy pill and Foundayo are labeled for weight management; Ozempic and Mounjaro for type 2 diabetes. Ask your prescriber what your plan requires; prior authorization is common.
  2. Self-pay direct from the manufacturer. Novo Nordisk sells Wegovy, Ozempic and the Wegovy pill through NovoCare Pharmacy; Lilly sells Zepbound and Foundayo through LillyDirect. Prices change often, so check the manufacturer's page before you decide.
  3. Through an online weight-loss clinic. A licensed clinician reviews your history by video or questionnaire, prescribes if appropriate, and either sends the prescription to a pharmacy or ships treatment to you. This is the fastest route if your regular doctor will not prescribe for weight loss.

The Part No Prescription Fixes

Every drug on this page works the same way at the dinner table: it shrinks your appetite until three or four bites feel like a meal. That is where the weight loss comes from, and it is also where the muscle loss comes from if those bites are toast and crackers instead of chicken and eggs. The medication does not care what you eat. Your body does.

HealthyOne was built for exactly this. Say what you ate or snap a photo, and it logs the protein before you have cleared the plate. GLP-1 mode raises your protein target to protect muscle while the weight comes off, the dashboard shows protein first, and the 50-plus nutrient tracker underneath flags the iron, fiber and B12 that slide when every meal is tiny. Start with the GLP-1 protein food list and the 7-day GLP-1 meal plan.

FAQ

Which GLP-1 causes the most weight loss?

In trials, tirzepatide (Zepbound, Mounjaro) has produced the largest average loss: about 20 percent of body weight at the highest dose over 72 weeks, and 20.2 percent versus 13.7 percent for Wegovy 2.4 mg in the SURMOUNT-5 head-to-head trial. The new Wegovy HD 7.2 mg dose closed much of that gap at about 20.7 percent in STEP UP. Individual results vary widely.

Is Ozempic the same as Wegovy?

Same molecule, semaglutide, from the same company. Ozempic is approved for type 2 diabetes with a maximum dose of 2 mg weekly. Wegovy is approved for weight management with doses up to 2.4 mg, and since March 2026 a 7.2 mg HD option. Insurance usually follows the label, which is why Ozempic for weight loss alone is often denied.

Which GLP-1 is cheapest without insurance in 2026?

The pills. The Wegovy pill starts at $149 a month for the 1.5 mg dose and tops out at $299. Foundayo starts at $149 and tops out at $299 to $349 depending on refill timing. Injections run $349 (Wegovy) to $449 (Zepbound at the higher doses through LillyDirect). Prices change often, so confirm before deciding.

Are the pills as effective as the shots?

Not quite, on average. The Wegovy pill produced 13.6 percent loss in OASIS 4 across everyone enrolled, and Foundayo about 11 percent in ATTAIN-1, versus roughly 15 percent for Wegovy 2.4 mg and 20 percent for Zepbound. For many people the convenience and price make the pill the one they will actually stay on, which matters more than the trial average.

The Bottom Line

If maximum weight loss is the goal and cost is not the barrier, tirzepatide (Zepbound) has the strongest trial numbers. If you want a weekly shot at a lower cash price, Wegovy at $349 or Wegovy HD at $399 is the direct competitor. If you will not do injections or want the lowest monthly cost, the Wegovy pill and Foundayo now make a daily tablet a real option at $149 to $299. Ozempic and Mounjaro are the same molecules labeled for type 2 diabetes, which matters mostly for insurance. Whatever you and your prescriber choose, the eating rules are identical: small portions, protein first, and track it.
